Student Athlete Information

The Sports Medicine staff would like to welcome all prospective student athletes to the University of San Diego. Our department operates under the supervision of the University of San Diego's designated team physician, the Associate Director of Athletics for Sports Medicine, Associate Athletic Trainers, and Assistant Athletic Trainers. The Sports Medicine facility and the certified athletic training staff are here exclusively to care for the intercollegiate athletes' injuries and/or illnesses.

Student athletes must be cleared each year by the sports medicine department for participation. The medical history and insurance information forms required by all student athletes for clearance are below. New and returning athletes must complete updated forms each year, and the insurance information form must be signed by the parent(s) regardless of age.

Print out the appropriate forms (as indicated below), complete, sign, and return them to the sports medicine office indicating your name and sport:
